Information You Choose to Provide
When you request a consultation, the website asks for your name, phone number, email address, preferred therapy setting, and permission to contact you about your request. Other contact forms may include a message field. Please do not enter diagnoses, medical history, insurance identification numbers, or other sensitive clinical details in website inquiry forms.
How Inquiries Reach Our Team
Website inquiry forms use FormSubmit to send information by email to Kori's administrative team for follow-up. These forms are for general contact requests, not clinical records or a patient portal. Review FormSubmit's privacy information for its handling of information. Ask our team about the appropriate intake process before sharing medical information.
Hosting, Fonts & Maps
Vercel hosts this website. Hosting and network providers may process technical information such as IP addresses, requested URLs, browser information, and access times to serve and protect the site. Fonts load from Google Fonts. The Contact page includes a Google Maps embed; map links open Google's services, where their privacy terms apply.
Analytics & Advertising
This website does not currently load Google Analytics, Google Tag Manager, advertising pixels, or remarketing tags. Landing-page code does not send form entries to analytics or create advertising audiences. Campaign parameters and ad click identifiers may appear in the address of a link you follow and in hosting requests. The landing-page code does not copy them into form submissions or store them in cookies or browser storage.
External Intake & Email Links
The English and Spanish client intake buttons open forms hosted by Zoho. Information entered there is handled through that separate service. Careers applications are downloaded from this website and returned by email. Review the information on those services and contact Kori's with questions before submitting sensitive material.
Adults & Privacy Questions
Website inquiry forms are intended for adults, including parents and guardians. Do not submit a child's personal information through a general inquiry form.
